Job Title: Physical Therapist (PT) Home Health Profession: Therapy/Rehabilitation Specialty: PT Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum years of experience required License: Valid physical therapist license required Certifications: Appropriate certifications required Must-Have: Previous charge experience preferred Must have a solid understanding of patient care protocols and documentation processes. Description: Full-time role includes working Saturday and Sunday, plus one weekday each week. Position involves providing physical therapy services in a home health setting. Therapists will work with a variety of patient age groups and diagnoses. Candidates should be proficient in charting and documentation. Experience with interdisciplinary support is highly valued. Demonstrated ability to work independently and manage a caseload effectively is essential. Candidates must be committed to patient-centered care and rehabilitation.
